Responsibilities:

  • Deliver physical therapy services to patients following a physician's treatment plan and in accordance with the state Physical Therapy Practice Act.
  • Provide guidance and education on treatment procedures to clients, family members, and staff.
  • Assist in evaluating patient’s functional capabilities.
  • Contribute to the development of personalized care plans.
  • Document clinical progress and submit notes regularly.
  • Convert exercises into functional activities that promote daily living skills.
  • Design modifications for equipment and the home environment as needed.
  • Maintain regular communication with attending providers regarding patient progress and overall condition.
  • Educate clients and families about the treatment process and motivate them for progress.
  • Prepare clinical summaries assessing goal achievements.
  • Supervise physical therapy assistants in accordance with organizational guidelines.
  • Ensure accurate clinical record-keeping that tracks client progress.

Qualifications:

  • Licensed Physical Therapist with a degree from an accredited School of Physical Therapy.
  • Valid physical therapy license in the state of practice.
  • Minimum of two years of experience in an acute hospital setting or similar (preferred).
  • Current CPR and First Aid certification.
  • Reliable transportation, valid driver's license, and appropriate automobile insurance required.
