Clinical Care Coordinator, FOH, Greenfield

CSO is looking for a Clinical Care Coordinator for our team at Friends of the Homeless in Greenfield.

The Clinical Care Coordinator conducts biopsychosocial assessments and Individual Service Plan development with the client and the Case Manager and will coordinate substance abuse and mental health treatment services, case management, supports and projects activities including training, communication, referrals, and integrated services (e.g. dual diagnosis).

Starting Salary: $50,000-$63,000/year

As a Clinical Care Coordinator:

You Will:

  • Complete comprehensive biopsychosocial assessment
  • Participate in early and ongoing team collaboration with case managers and outreach workers to ensure program participants' needs are identified and coordinated
  • Develop and implement an individual service plan (ISP) in collaboration with the individual collaterals
  • Develop a risk management/safety plan in collaboration with the individual and collaterals
  • Facilitate care planning team (CPT) meetings
  • Identify and develop of natural supports
  • Provide substance abuse and Behavioral health education, advocacy and support
  • Provide therapeutic support and services for uninsured and underinsured individuals
  • Provide on-site psychoeducational groups related to specific BH/SA diagnoses and issues
Your Background:
  • Master's degree in a behavioral health discipline or related field (required)
  • Clinical license (preferred)
  • Valid driver's license and reliable transportation required
