Family Medicine Physician: Coordinated and Correctional Care Medical Director

Location: CentraCare - Coordinated Care Clinic, 3701 12th St N Ste 102, St. Cloud, MN 56303
FTE: Flexible, 0.4-1.0

Step into a meaningful leadership role at CentraCare, where your expertise will shape the delivery of care across our communities. In this position, you will co-lead the Coordinated Care Clinic and Correctional Care program. Leadership of these programs will provide you with a unique and rewarding opportunity to care for patients with significant social drivers of health. This role will partner with law enforcement, courts, and community agencies to improve care delivery and community outcomes.

What You'll Do
  • Serve as Correctional Medical Director, overseeing inmate medical and mental health care, supporting jail operations, and leading clinical protocols and community engagement.
  • Provide compassionate care to patients in our Coordinated Clinic, practicing on a team of dedicated professionals with diverse backgrounds.
  • Co-lead the Chronic Controlled Substance Monitoring Program, shaping system- and community-wide chronic opioid management and prescribing practices, while leading and participating in key committees.
  • Lead and partner with CentraCare leaders to drive strategic initiatives, support and mentor providers, and foster a high-performing, collaborative culture.
Must be BE/BC in Family Medicine or Internal Medicine. Experience working within Addiction Medicine required. Must be able to obtain a MN License.

Pay and Benefits
  • Starting pay begins at $260,000 per year; exact wage determined by years of related experience
  • Salary range: $260,000 - $290,000 per year
  • Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
