You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies, with hours, responsibilities, and compliance obligations tracked separately for each.

This role is structured as a 24-hour per week position, with approximately one-third of the total weekly hours (about 8 hours per week) allocated to each organization. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care, each employer will manage its own onboarding, HR, and compliance processes in accordance with applicable laws and organizational policies.

ACCOUNTABLE TO: Operations Manager

CLASSIFICATION:

This is a part-time, W-2 employee position totaling 24 hours per week, worked over three scheduled days, with hours allocated across three affiliated companies as follows:

    Part-time, W-2 employee - approximately 2.4 hours per week (10% of total weekly hours)

This role is one of five individuals collectively assigned to meet and exceed the COMAR-mandated leadership coverage requirements, which include:
  • 20 hours per week for a Program Director at Community Wellness OMHC
  • 20 hours per week for a Medical Director at Community Wellness OMHC
  • 20 hours per week for a Clinical Director at Lighthouse Behavioral Health Center
Collectively, the five assigned professionals provide a total of 120 hours of leadership coverage per week, which exceeds the COMAR requirements for each organization as follows:
  • Community Wellness OMHC (Backup Program Director Requirement):
    COMAR requires 20 hours/week; the team provides 24 hours/week
    → Exceeds requirement by 4 hours/week
  • Community Wellness OMHC (Medical Director Requirement):

This structure ensures robust clinical leadership and regulatory compliance across all entities, with ample coverage to support oversight, team collaboration, and quality improvement initiatives.

WORK SCHEDULE:

The Psychiatric Mental Health Nurse Practitioner (PMHNP) will work every Monday, Tuesday and one other set weekday each week, from 8:00 AM to 5:00 PM on each assigned day. The schedule will include one full administrative day (Tuesdays) and two days (Tuesday and one other set weekday) focused primarily on direct client care and clinical services.
  • Tuesday of each week is designated as the Administrative Day, with structured responsibilities that include leadership meetings, documentation audits, quality reviews (COMAR, CARF, policies), psychiatric updates (90792), and team collaboration.
  • The Tuesday and one other set weekdays will focus on the provision of psychiatric services, including:
    • Psychiatric focused evaluations and medication management
    • Substance use disorder focused evaluations and medication management
    • Primary care focused evaluations and medication management
    • Care coordination and supervision of multidisciplinary clinical staff
    • Documentation, prescribing, treatment planning, and compliance with CWOMHC / LHBHC / FHFGC service standards
COMPENSATION RANGE:
Compensation for this position ranges from $30.00 to $100.00 per hour, and is commensurate with experience, expertise, verified credentials, and available company budget.

DUTIES AND RESPONSIBILITIES:
1. Facilitate and document coordination with external primary care providers, as clinically appropriate and with proper consent.
2. Prescribe medications following a full face-to-face evaluation and assess for any contraindications prior to initiating a medication regimen.
3. Educate clients and, when appropriate, caregivers on the purpose, effects, and risks of prescribed medications.
4. Monitor and adjust medication regimens as clinically indicated; conduct evaluations at a minimum every 90 days for ongoing medication management.
5. Notify the client's primary care provider of new prescriptions or medication changes every six months or as clinically required.
6. Conduct peer reviews of other prescribers' work and support documentation compliance across the clinical team.
7. Participate in interdisciplinary team meetings with HR, Scheduling, Patient Access, and other departments to support clinical and administrative needs.
8. Conduct batch documentation audits, including review and signature of referrals, treatment plans, biopsychosocial, ASAMs, and intern notes.
9. Collaborate with external agencies or treatment providers in cases involving shared clients or complex care needs.
10. Accept and review incoming lab results in ICANotes; use clinical judgment to adjust care plans accordingly.
11. Ensure all documentation is completed within expected timeframes using ICANotes and is coded appropriately using evaluation and management CPT codes.
12. Contribute to quality assurance, audit readiness, and performance improvement initiatives to ensure compliance with COMAR, CARF, Medicaid, and internal standards.
13. Provide support and education to clinical staff related to medication protocols and service delivery expectations.
14. Submit periodic reports and summaries related to administrative and clinical oversight duties.

15. Administrative Leadership - Tuesday (8:00 AM - 5:00 PM)

During clinical days, the PMHNP will provide psychiatric care, medication-assisted treatment (MAT), and integrated primary care coordination across the following affiliated programs:

The schedule for these dedicated clinical days include:
  • Conducting psychiatric evaluations, re-evaluations, and follow-up medication management visits for individuals receiving outpatient mental health, substance use disorder, or primary care services
  • Delivering Medication Assisted Treatment (MAT) using FDA-approved medications such as buprenorphine, in alignment with COMAR 10.09.80 and federal MAT standards
  • Responding to medical emergencies in outpatient or residential care settings and conducting diagnostic assessments as clinically indicated
  • Managing and documenting all services in the electronic health record (EHR) using appropriate billing codes, including:

1. Direct Clinical Care & Medication Management

The Psychiatric Mental Health Nurse Practitioner (PMHNP) at Lighthouse Behavioral Health Center serves a dual role as both a clinical provider and program leader within a licensed behavioral health and residential substance use disorder treatment setting. SCHEDULED DUTIES AND RESPONSIBILITIES:

1. Medication-Assisted Treatment & Psychiatric Services
  • Conduct initial psychiatric and diagnostic evaluations, including history-taking, risk assessment, and treatment planning
  • Prescribe and manage FDA-approved medications (e.g., buprenorphine), ensuring dosage is personalized and clinically justified
  • Monitor and manage withdrawal symptoms, support stabilization, and prevent relapse through individualized MAT plans

3. Clinical Leadership (Clinical Director Role per COMAR 10.21.07.02(14))

  • Perform patient assessments, obtain medical histories, conduct physical exams, and order diagnostic tests as needed
  • Diagnose and treat common acute and chronic conditions such as hypertension, diabetes, asthma, infections, and minor injuries
  • Prescribe medications and develop individualized care plans in line with current clinical guidelines
  • Deliver preventive health screenings and educate patients on lifestyle changes, immunizations, and disease prevention
  • Coordinate care with internal behavioral health, substance use, and case management teams as appropriate
  • Make referrals to specialty providers, hospitals, or community-based resources when medically indicated
  • Document all encounters accurately in the electronic health record (EHR) and ensure compliance with regulatory and billing requirements
  • Participate in team huddles, case reviews, and clinical quality improvement initiatives
  • Serve as a clinical resource to support staff and promote evidence-based practice within the care team
