Geriatrics Nurse Practitioner

A reputable Skilled Nursing Facility in El Paso, TX is seeking a dedicated and experienced Nurse Practitioner (NP) to join their team in a full-time, permanent capacity. This role is ideal for an NP who is passionate about geriatric care and skilled nursing environments, and who thrives in a collaborative, patient-centered setting.
Key ResponsibilitiesProvide primary and acute care to SNF residents
Perform daily rounds, assessments, and follow-up visits
Diagnose and manage chronic and acute conditions
Develop and monitor individualized care plans
Coordinate care with physicians, nurses, therapists, and family members
Order and interpret diagnostics, labs, and imaging
Ensure accurate documentation in the EMR
Participate in quality initiatives and interdisciplinary meetings
Full-time, permanent role
Monday Friday (hours determined by facility)
No call or weekends required (if facility confirms)
Active Texas Nurse Practitioner license
Experience in Skilled Nursing Facilities or Post-Acute Care required (minimum 1 2 years)
Strong understanding of geriatric care and chronic disease management
Ability to work independently with physician support as needed
Current BLS/ACLS
Strong communication and teamwork skills
