Join our Team as a Home Care Physical Therapist Experience the freedom, autonomy, and flexibility of providing one-on-one patient care in the comfort of their homes.
Work Schedule: Full-time, Monday - Friday, with a weekend rotation approximately every 6 weeks depending on staffing levels.

- Annual salary range: $90,000 - $110,000 plus additional compensation for weekend rotations.

Key Qualifications:
- Must currently be licensed as a Physical Therapist in the state of Minnesota.
- Preferably, two years of relevant experience as a Physical Therapist within the last five years.
- Must be able to travel within the geographic area serviced by our office; a valid driver's license, reliable transportation, and proof of auto insurance are required.
- CPR/BLS certification is essential.
Your Responsibilities:
- Collaborate with physicians to evaluate patient function, set treatment goals, and develop personalized care plans.
- Engage with patients, their representatives, and caregivers to assess progress and modify treatment plans as needed.
- Monitor changes in patient status during visits and consult with physicians about necessary adjustments to the care plan.
- Communicate effectively with the interdisciplinary healthcare team to ensure coordinated patient care and discharge planning.
- Maintain timely, thorough, and accurate documentation of patient progress and treatment.
