Your Responsibilities
- Conducting thorough patient evaluations, coordinating diagnostic tests, and crafting personalized care plans
- Diagnosing and managing acute, episodic, or chronic health conditions in conjunction with physicians or independently as appropriate
- Providing education to patients and their families around preventive measures, wellness, and effective disease management
- Prescribing medications and therapies in accordance with your scope of practice
- Engaging in patient follow-ups and chronic care oversight
- Collaborating with multidisciplinary teams to assure cohesive and comprehensive care
- Maintaining accurate and complete electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Graduation from an accredited Nurse Practitioner program
- Active Nurse Practitioner licensure in the state of California
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or specialty equivalent
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to secure within 90 days)
- Robust clinical skills paired with a caring demeanor
- Strong communication abilities with patients, families, and healthcare colleagues

- Competence in EMR systems and ease with telehealth technologies
