This full-time position operates on weekdays, Monday through Friday from 8 AM to 5 PM, with a limited after-hours on-call rotation. This role will involve caring for patients in the Kalamazoo County area.

What You'll Do as a Hospice Nurse Case Manager:

  • Serve as the primary point of contact for patients and families, guiding them through their entire hospice journey.
  • Provide and document skilled nursing care tailored to each patient’s individualized Plan of Care in various settings, including their home or assisted living facilities.
  • Conduct initial and ongoing assessments to monitor patient conditions and adjust care as necessary.
  • Collaborate with an interdisciplinary team to develop and refine personalized care plans.
  • Offer emotional, physical, and spiritual support not only to patients but also to their loved ones, providing education, comfort, and bereavement guidance.
  • Educate caregivers on how to safely and confidently care for their loved ones, providing practical training and emotional support.
  • Communicate significant updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
  • Maintain accurate and current clinical documentation while adhering to infection control and compliance standards.
  • Engage in quality improvement initiatives to guarantee that care remains consistent, compassionate, and efficient.
  • Assist patients and families in understanding crucial topics, including:
    • Medication administration
    • Hospice philosophy and services
    • Symptom and pain management
    • End-of-life processes and expectations

About You: Qualifications - What You'll Bring:

  • Active RN license in the state of employment (or eligibility to obtain).
  • At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
  • A genuine passion for hospice care, characterized by compassion, empathy, and patient-centeredness.
  • Comfortable providing care in diverse settings, including private homes and healthcare facilities.
  • Strong communication and critical thinking skills in emotionally charged situations.
  • Valid driver's license, auto insurance, and reliable transportation.
  • CPR certification is required.

Preferred Experience (Not Required):

  • Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
  • Admissions RN, wound care, float pool, or travel nursing experience.
  • Experience with terminally ill patients or serious illness support.
