Inpatient Dialysis Social Worker

PRIMARY PURPOSE: You'll be responsible for conducting psychosocial screenings and assessments, implementing care plans, and ensuring patients have the necessary resources and support services.
MINIMUM REQUIREMENTS:
- Education: Master's degree in social work.
- Experience: None required.
- Certification/Registration/Licensure: Must be licensed by the Texas State Board of Social Work. Obtain Heartsaver CPR certification and Satori Alternatives for Managing Aggression (SAMA) certification within specified time frames.
Skills or Special Abilities:
- Effective communication with diverse patient populations, medical staff, and community agencies.
- Knowledge of community resources to meet patient needs.
- Excellent interpersonal and counseling skills.
- Demonstrate patient-centered behaviors.
KEY RESPONSIBILITIES:
- Conduct psychosocial screenings and assessments to identify patient needs and coordinate interventions.
- Provide counseling to patients and families about the emotional, social, and financial impacts of illness.
- Participate in discharge planning to arrange post-hospital resources.
- Communicate with the multidisciplinary team regarding discharge planning and patient status.
- Handle cases of abuse, domestic violence, and other critical social issues with appropriate interventions.
- Maintain thorough documentation of assessments and interventions.
- Manage referrals and ensure efficient communication among various stakeholders.
- Stay compliant with regulations and guidelines pertinent to the social work field.
- Cultivate positive relationships with management, vendors, and community agencies to support patient needs.
- Submit and report timely and accurate data as required.
- Continuously seek improvements in work processes for enhanced customer satisfaction.
- Engage in professional development and stay updated on Social Work best practices.
- Participate in committees and peer reviews to foster quality and patient satisfaction.
