Job Summary:

As a Licensed Practical Nurse (LPN), you will provide quality, cost-effective direct care for patients and families. Under the guidance of a Registered Nurse, you will collaborate with the healthcare team to help patients achieve their care goals.

Your responsibilities include:

  • Collecting health information and communicating important details to the physician and healthcare team
  • Assisting in implementing the physician's plan of care and providing patients with essential health education
  • Maintaining laboratory results, continuous glucose monitoring statistics, and preparing charts
  • Delivering patient care in an outpatient environment, including the Endocrine Clinic and Diabetes Education program
  • Support with administrative tasks such as patient registration and handling incoming calls professionally

Qualifications:

  • Current Connecticut LPN license

Minimum Requirements:

  • Experience in an outpatient or ambulatory care setting
  • Excellent customer service skills
  • Effective written and verbal communication skills
  • Independent problem-solving abilities
  • Bilingual in English and Spanish preferred
