What You’ll Do:
-
Engage and collaborate with Referral Partners to secure all required information and documentation.
-
Leverage all available resources to help our customers understand and navigate the home healthcare referral process.
-
Provide assistance to customers telephonically, electronically, or by other organization-approved means.
-
Receive and document referral information and medical orders.
-
Assign diagnosis codes and process referrals in a timely manner to appropriate service line for admission.
-
Collect and document information about the patient’s situation and functional capacity, relative to the care plan post discharge.
-
Maintain patient records in accordance with established protocols and policies.
-
Friday-Monday Schedule, four 10-hour shifts.
What You’ll Bring:
-
Active RN license in the State of Maine.
-
2 years recent RN experience.
-
Basic computer knowledge and data entry experience.
Hours: 9:00am - 5:30pm. Thursday- Monday. Must participate in a holiday rotation.
