Job Title: | Rehab Specialist (Health Home Supervisor) | Job Category: | Clinical Salary | $25-$35 based on Licensure and Experience- Days Required: 40 hours per week. Monday (Telework), Tuesday-Friday in Office 8:30 am-4:30 pm

Job Description The Rehab Specialist has the responsibility of providing education and training to adults with serious mental illness aimed at developing problem-solving skills, enhancing self-esteem, developing pre-vocational skills, improving social/relationship skills, and facilitating enhanced community integration and recovery within a psychosocial rehabilitation setting. The Rehab Specialist is responsible for documentation according to agency policy and Medicaid and Licensure regulations including service documentation and development and review of psychosocial rehabilitation plans. Advocating for persons with mental illness within the service sector and the community at large is a major component of this position. In carrying out position responsibilities, the Rehab Specialist reports to and receives supervision from the Clinical Supervisor or Chief Operating Officer.

Roles & Responsibilities:

Complete biopsychosocial/functional assessments and develop individualized rehabilitation plans for adults and minors who suffer from a variety of mental health challenges.

Provide psycho-educational activities and conduct groups to teach persons served about mental health, wellness, self-help, and appropriate medication to avoid complications and relapse.

Provide supervision to direct care staff by conducting individual and /or groups supervision to Direct Service Workers

Communicates with Behavioral Health colleagues, Primary Care providers, therapists and other providers and agencies as needed to facilitate integrative care.

Monitor weekly/monthly notes, weekly/monthly collaboration notes, contact notes, client visit logs for any on-site programming.

Identify and facilitate enrollment of all eligible clients into the program, via ongoing collaboration with the agency intake coordinator, clinical supervisor, and psychiatrist. Assist the ED and COO in identifying and marketing to relevant referral sources that could benefit from PRP.

Effectively respond to client emergencies and/or urgent needs.

Monitor client satisfaction of services and ensure Direct Service Workers possess and utilize strong clinical/rehabilitative skills in delivery of services.

Maintain consistent and open communication with the clinical staff on all relevant and pertinent clinical and administrative issues related to the program.

Educate direct service staff (Direct Service Workers) on how to provide rehabilitation services that correlate to the goals identified in the individual’s IRP and meets COMAR requirements.

Assist all new and existing DSW’s with learning proper documentation protocols.

Train all Direct Service Staff on the required documentation standards and associated timelines set forth by COMAR regulations and CARF conformance standards. Such documents include but are not limited to, Screenings, Rehabilitation Assessments, Individual Rehabilitation Plans (IRP’s) and Encounter Notes.

Provide ongoing training and support to direct service staff related to the development and implementation of individual and group interventions that are tailored to the specific needs of the client(s).

Ensure that rehabilitation services are sufficiently meeting the needs of individuals in the program on an ongoing basis.

Maintain regularly scheduled group and individual supervision with all direct care staff that include relevant discussions and updates regarding both administrative and clinical issues.

Enforce and comply with electronic documentation entry and approval timelines set forth by the agency.

Ensure that documentation is timely and reflects progress toward treatment plan goals or barriers to their achievement, in specific, concrete language.

Attend trainings on relevant topics such as recovery, medication management and psychiatric rehabilitation.

Be aware of and follow program policies and procedures including those regarding confidentiality.

Participate in quality improvement and data gathering as assigned.

Competencies: Knowledge of:

The chronically mentally ill.

Principles/techniques of psychosocial réhabilitation & psychotropic medicationsPrinciples of recovery and community integration.

Dual disorders.

Principles/techniques of individual, family, and group counseling; intra- and inter-agency coordination; local human service agencies; principle/techniques of case management/follow-up services; and food service and related sanitation.

Skills(s) in:

Implementing the daily operation of a psychosocial/day support program, which includes planning, organizing, and implementing daily activities.

Providing case management/follow up services, coordinating consumer services.

Providing supportive counseling services to the specified population.

Verbal, written and interpersonal communications; supervising, organizing, and motivating participation in all phases of psychosocial programming.

Maintaining accurate documentation.

Ability to:

Manage and maintain an active caseload.

Utilize technology in everyday work duties.

Intervene with clients in crisis situations according to agency protocols.

Work effectively with all types and/or levels of people; maintain working relationships.

Qualifications Certified by the Commission on Rehabilitation Counselor Certification, or bachelor's prepared certified psychiatric rehabilitation practitioner currently certified by the U.S. Psychiatric Rehabilitation Association.

Master’s Degree in Social Science or related field.

Licensure in the Social Science and/or related discipline (LGPC/LCPC or LGSW/LCSW).

Two (2) years of experience in administrative management within a behavioral health setting.

Two (2) years’ experience working with minors and adults with behavioral health diagnoses and issues with functioning in major life domains.

Fingerprint clearance, and pass drug test, physical exam, and clear TB test.

Must be able to pass full police/FBI background check.

Must be able to pass a Child Protective Clearance.

Physical & Environmental Requirements:

The position may require you to occasionally stoop, kneel or crouch.

Must be able to occasionally lift 10-15 lbs.

This position requires that the employee sit for 5-8 hours and stand for 2-3 hours.

Must be able to follow all safety rules related to treatment.
