We are excited to announce an opportunity for a Clinical Pharmacist Specialist to join our growing Specialty Pharmacy Clinical Services team This role is vital in providing excellent patient care through telephonic consultations and disease management guidance.

As a Clinical Pharmacist Specialist, you will engage with patients over the phone to discuss their specialty medications, perform medication reconciliations, screen for potential drug interactions, and offer side effect management recommendations. Your expertise will be essential in interpreting prescriptions, clarifying with prescribers, and facilitating the prompt processing of orders. We're looking for a proactive individual who will meet production metrics, contribute to quality standards, and promote an exceptional patient experience.

How you will make a difference:

  • Conduct telephonic patient education consultations in alignment with our Patient Management Program, focusing on disease state management, side effect strategies, medication administration, and appropriate storage conditions.
  • Document consultations and clinical interventions thoroughly in our pharmacy software system, adhering to internal policies and external regulatory guidelines.
  • Critically evaluate and interpret specialty medication prescriptions, ensuring accuracy and compliance by contacting providers and patients as necessary.
  • Work collaboratively with healthcare providers to discuss new prescriptions, resolve pharmaceutical inquiries, and facilitate prescription transfers.
  • Be a resource for the pharmacy team by answering clinical questions and verifying prescription information in the order entry system.
  • Stay updated on specialty pharmacy advancements, including new therapies and treatment guidelines, while adjusting to new workflows efficiently.
  • Provide mentorship and training to less experienced pharmacists and act as a backup when required.
  • Document and report any medication errors in accordance with company policies to enhance quality and prevent future issues.
  • Uphold federal and state laws, alongside the ethical standards that define our organization.
  • Perform additional duties as needed.

What we are looking for:

  • A degree from an ACPE Accredited School of Pharmacy (B.S. Pharmacy or PharmD).
  • A current, unrestricted pharmacist license in your state of residence, with ongoing educational credits to maintain it.
  • Preferred experience of at least two years as a licensed practicing pharmacist.
  • A strong clinical background is crucial.
  • Familiarity with Specialty Pharmacy medications is a plus.
  • A commitment to enhancing compliance with program objectives.
  • Strong collaboration and interpersonal skills to work respectfully with colleagues.
