Serenity Hospice is seeking a compassionate and dedicated Hospice Social Worker to join our caring and supportive team. This role is essential in providing emotional support, guidance, and resources to patients and their families as they navigate end-of-life care. We are looking for someone who is empathetic, professional, and committed to delivering care with dignity and respect.

Key Responsibilities:
  • Conduct psychosocial assessments and provide ongoing emotional support
  • Develop individualized care plans with the interdisciplinary team
  • Assist families with resources, advance directives, and community services
  • Provide crisis intervention and bereavement support
  • Maintain accurate and timely documentation
Requirements:
  • Bachelor's or Master's degree in Social Work (MSW preferred)
  • Valid state Social Work license
  • Hospice or healthcare experience preferred
  • Strong communication and interpersonal skills
  • Compassionate, patient-centered approach
