Your Role as a Home Health Occupational Therapist:

  • Provide targeted occupational therapy to assist patients in overcoming challenges related to daily tasks due to illness or injury
  • Collaborate with a dedicated home health team including RNs, LPNs, CNAs, HHAs, PTs, and SLPs to focus on each patient's personalized plan of care and goals
  • Assess patient needs, identify deficits, establish achievable therapy goals, and diligently document progress
  • Guide patients through exercises aimed at improving fine motor skills and coordination
  • Recommend adaptive equipment like grab bars and shower chairs to enhance safety and support
  • Evaluate fall risks and implement strategies to increase home safety
  • Educate patients and families on the therapy plan, exercises, goals, and self-care techniques
  • Supervise and mentor Certified Occupational Therapy Assistants in providing therapy

Qualifications We're Looking For:

  • Graduated from an accredited Occupational Therapy Program with an active OT license in Colorado
  • A minimum of one (1) year of experience in occupational therapy within home healthcare
  • CPR Certification required (must demonstrate in a course)
  • Pass a background check and drug test (testing for THC is included)
  • Familiarity with state and federal home health regulations
  • Possess strong clinical judgment, excellent interpersonal skills, and a compassionate, resourceful attitude
