ChristianaCare HomeHealth is seeking a Full Time Medical Social Worker. The successful candidate will support Dover, Smyrna, Frederica and the surrounding areas; work day shift hours 8:30am-5pm, and an occasional weekend day or holiday based upon departmental needs.

Duties and Responsibilities:

• Participates in Case Conferences and the development of total plans of care with the multidisciplinary team.
• Provides clinical social evaluations and plan appropriate interventions based upon evaluating findings.
• Identifies patients appropriate for specialty programs and Telehealth.
• Completes home visits to clients and discussions evaluation findings with Senior Medical Social Worker
• Identifies appropriate community resource referrals to address the client's and the family's practical and environmental needs.

Requirements:

• Master's Degree from a school of social work accredited by the council on Social Work Education

• 2 or more years experience in health care setting required

• Prefer 1 year of home care or community field experience

Hourly Pay Range: $27.31 - $40.96 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
