View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, as well as other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and supports. The services provided by View Point Health contribute to the reduction in homelessness, reduction in crime and subsequent incarcerations, higher graduation rates, lower dropout rates, decreased ER visits and decreased inpatient admissions.
Services are client-centered, trauma informed and recovery-focused including behavioral health assessment, psychiatric evaluation, medication management, group and individual therapy, family therapy, peer support, psychosocial rehabilitation, assertive community treatment, community based case management, high fidelity wrap around, housing, supported employment, court services, crisis intervention, on-site pharmacy services, rehabilitation services, brief residential crisis stabilization services, short-term substance abuse residential treatment as well as developmental disability day services and group homes.
As a partner in SCIP, VPH staff along with local law enforcement, District Attorney office, Gwinnett Courts and Juvenile Court, Gwinnett County Public Schools (GCPS) and Northside Hospital System will aid those impacted by gun violence and perpetrators of such violence, where appropriate. The program will be primarily focused on the mental health needs of victims of gun violence in Gwinnett County.
Duties & Responsibilities:
- Provides professional therapeutic services to persons with mental illness and/or addictive disease.
- Arranges appointment schedule at optimal intervals to maximize use of work time.
- Conducts face-to-face comprehensive assessment to identify problems and service needs.
- Writes individual service plans that meet Medicaid, insurer, CARF, and other required standards for diagnoses, content, justifications for treatment, evidence of consumer input, and treatment time frames.
- Writes service plans that are clear, concise, complete, grammatically correct, timely, and properly coded.
- Secure or provide necessary signatures required to secure authorization for services. Follow all Medicaid, state, insurer, and other regulations related to the credentials required to provide authorized services.
- Provides individual, family or group therapeutic counseling and/or educational and skill development sessions as indicated in the service plan.
- Monitors consumer progress and compliance with the service plan.
- Writes timely progress notes that relate directly to the treatment outcomes described in the service plan.
- Assembles, analyzes, and interprets new/updated information about the consumer in order to reassess treatment needs.
- Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated.
- Requests treatment authorizations and re-authorizations within required time frames.
- Provides and bills only those consumer services that are authorized in the service plan.
- Provides crisis intervention services to existing and walk-in consumers as needed.
- May provide on-call, after-hours services on a rotational schedule.
- May serve as case manager to facilitate and ensure communication, linkage, and continuity of care between consumers, physicians, other organizational components, community resources and service providers.
- Establishes linkages and coordination of services with community agencies.
- Develops and maintains cooperative working relationships with community service providers. Informs consumers and families of the availability of community advocacy and support groups.
- Other related duties as assigned.
- Provide individual and group therapy using DBT (Dialectical Behavior Therapy) and Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) to clients affected by gun violence or gun related death in the community.
- Conduct clinical assessments, develop treatment plans, and monitor progress.
- Deliver immediate and individualized mental health support to victims of gun violence, including crisis intervention.
- Maintain timely and accurate clinical documentation per legal and ethical standards.
Master's degree in in social work and counseling from an accredited college or university and eligibility to be licensed and two (2) years of supervised experience in the are of assignment or two (2) years of experience at the lower-level Social Worker, Licensed 1 (HCP050) or position equivalent.
Note: Some position may require licensure in the stat of Georgia as an LMSW, LAPC, LAMFT, LCSW, LPC, or LMFT.
Preferred Qualifications:
- Full Licensure (LCSW, LPC, or LMFT.
- Experience working with the Severe and Persistently Mentally Ill population, ACT teams, or individual identified as being frequent users of ERs, Psychiatric Hospitals, Crisis Stabilization Units, and the criminal justice system.
- Trained in trauma focused treatment, i.e. TF-CBT and DBT.
- Experience working with youth and adolescents.
- Experience working in a highly stressed, fast-paced environment with a very emotionally and physically changing population.
- Experience working in multi-disciplinary teams.
- Experience working with community partners and local jurisdictions.
- Ability to multi-task and manage time.
- Must be able to lift 20 pounds.
- Requires long periods of sitting/standing.
- Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
- Candidates for selection must pass a criminal background check (including fingerprinting).
- Must pass a pre-employment drug screen and subject to random drug screens.
