Position Overview:

As a Weekend Revisit LPN, you will use your skills and training to deliver personalized care in a fulfilling environment that grants you autonomy. Responsibilities include:

  • Facilitating communication between the patient and the interdisciplinary team.
  • Developing and maintaining a therapeutic relationship by providing education and fostering self-advocacy and independence.
  • Utilizing state-of-the-art technology to document skilled services provided.

Required Qualifications:

  • Valid CT LPN license.
  • Minimum of one year of LPN experience; homecare experience preferred.
  • Valid driver's license and reliable transportation.
  • Proficient in computer skills.
