Job Title: Social Worker (LCSW) Profession: Social Services Specialty: LCSW (Licensed Clinical Social Worker) Duration: 16 weeks Shift: Day Hours per Shift: 8 hours (09:00 - 17:30) Experience: 2 years License: CA License required Certifications: LCSW Must-Have: - Biopsychosocial assessment experience - Ability to educate patients and caregivers to facilitate discharge - Experience with documentation systems Description: The Social Worker (LCSW) will provide essential social services support to patients. Responsibilities include conducting biopsychosocial assessments. The social worker will educate patients and caregivers to facilitate smooth discharge planning. Documentation according to the established plan of care is required. Collaboration with interdisciplinary teams to ensure continuity of care is necessary. The ideal candidate will have experience with crisis intervention and supportive services. Candidates should be comfortable with completing assessments and developing care plans. Additional responsibilities may involve working with high-risk patient populations. Flexibility in scheduling and the ability to adapt to changing patient needs is required.
