Job Summary
Children's Memorial Hermann is seeking an Integrated Pediatrics Pharmacist, trained in residency, to join our team on a rotating schedule of 7 days on, 7 days off during the evening shift from 12 p.m. to 11 p.m. This role will primarily support our Emergency Department and Pediatric Intensive Care Unit (PICU). PALS Certification is preferred.

Minimum Qualifications
Education: Graduate of an accredited School of Pharmacy
Licenses/Certifications: Licensed as a Registered Pharmacist in the State of Texas; Pharmacy Sterile Product certification required if compounding sterile products.
Experience / Knowledge / Skills:
- Two (2) years of experience as a hospital Pharmacist, or at the discretion of the System Executive, System Pharmacy, with knowledge of unit dose medication and I.V. admixture processes.
Principal Accountabilities
- Fill and label prescriptions while providing drug-related information and education to inpatients, outpatients, and hospital staff.
- Dispense controlled substances and maintain accurate inventory records.
- Supervise pharmacy technical staff and mentor pharmacy students, residents, or new pharmacists.
- Participate in the Medication Use Evaluation Program and Process Improvement initiatives.
- Maintain medication profiles, address drug-related issues, and offer drug information consultations.
- Prepare I.V. solutions and additives, ensuring accuracy and safety.
- Report any medication errors or adverse drug interactions promptly.
- Assume full department management responsibilities in the absence of the Pharmacy Manager or Lead Pharmacist.
- Collaborate with other professionals to create newsletters and draft policies for review.
- Engage in clinical interventions, including medication profile reviews and therapeutic drug monitoring.
- Document clinical interventions accurately.
- Ensure patient and staff safety while adhering to all policies, procedures, and standards.
- Promote professional growth and development through required education and supporting departmental goals.
- Create compassionate, personalized moments for every community member and model Memorial Hermann's service standards.
- Perform other duties as assigned.
