Responsibilities
What You'll Do:
- Provide specialized therapy and wellness services (physical, occupational, or speech) for older adults.
- Conduct thorough assessments, develop personalized care plans, and implement evidence-based interventions in a collaborative team environment.
- Facilitate care coordination through wellness check-ins, effective communication with caregivers, and telehealth support.
- Uphold professional and ethical standards that align with company goals.
- Contribute actively to program development and continuous improvement initiatives.
- Empower residents to maximize their independence, confidence, and overall satisfaction.
