Job Description

The Linkage to Care Specialist conducts targeted outreach, recruitment, linkage and care coordination to assist people in accessing screening and linkage to care for HIV/AIDS, Hep C, STIs, PrEP, mental health and/or substance use disorder. Promotes the program directly to the community, as well as other CBO's and medical providers. Additionally, can promote overdose prevention, including Narcan training and distribution, testing/screening for HIV, PrEP, STIs, Hep. C and submit monthly reporting.

Job Tyle: Full Time

Pay: $22.00 - $24.00 per hour

Responsibilities:
  • Collaborate with the HIV, MAT, Medical and Behavioral Health Provider teams
  • Knowledge and experience working in the field of SUD, PrEP and/or HIV
  • Demonstrates cultural competency in working with diverse populations.
  • Maintains data and case records as required.
  • Facilitates enrollment in benefit and entitlement programs and referrals.
  • Conducts outreach to re-engage patients in care.
  • Performs job duties in compliance with systems and grant specific standards.
  • Participates in quality improvement trainings and activities to improve patient care.
  • Provides health education, risk reduction and adherence education to patients within the scope of discipline to assist patient in engagement and retention and reduce barriers to care to enhance health outcomes.
  • Evaluate social determinants of health to reduce barriers to timely access to care and treatment.
Requirements:
  • High School Diploma or GED
  • 1 year of relevant experience
