Verovian Nursing Agency is seeking an experienced and dynamic Manager of Care Management (RN or MSW) to lead the Care Management Team at a prestigious medical center in Fullerton, CA. Reporting to the Director of Care Management, this role is responsible for daily operations, staff oversight, and optimizing patient care coordination. The ideal candidate will have strong leadership skills, operational expertise, and a passion for improving patient outcomes.

- Oversee the daily operations and clinical practice of Care Management services.
- Lead and manage a team of Case Managers (RNs) and Social Workers (MSWs) to ensure effective case management.
- Ensure patients receive optimal health advocacy, access to care, and appropriate utilization of resources.
- Manage staffing, workflow efficiency, and resource allocation to optimize patient care coordination.
- Implement and monitor care management strategies to enhance patient experience and clinical outcomes.
- Work closely with physicians, nurses, and multidisciplinary teams to support seamless patient transitions across care settings.
- Ensure compliance with hospital policies, regulatory standards, and accreditation requirements.
- Support performance improvement initiatives and utilize data-driven strategies to enhance care management efficiency.
- Serve as a mentor and coach to staff, providing guidance, training, and professional development.
