We are seeking a dedicated Full-Time or Part-Time Speech Therapist / Speech-Language Pathologist (SLP) to provide exceptional care to our patients in their homes. You will have the flexibility to work in various counties, including Alameda, Contra Costa, Solano, San Joaquin, and Stanislaus.

Compensation:

  • Per Visit Rates: $140 - $220 per skilled speech therapy visit
  • Hourly Option: $50 - $65 (during office hours)

As a Speech Therapist, you will evaluate, plan, implement, and monitor therapy services tailored to the needs of each patient. Collaborating with nursing, physicians, caregivers, and therapy staff is essential to deliver high-quality, patient-centered care and help individuals reach their potential in daily activities.

Key Responsibilities: Evaluation & Assessment
  1. Conduct comprehensive assessments in the patient’s home for speech, language, voice, swallowing, and cognitive abilities.
  2. Review medical records and collaborate with nursing staff to consider patient history.
  3. Analyze the home environment for safety and support therapy success.
  4. Identify patient, family, and caregiver needs and goals.
Treatment Planning & Implementation
  1. Create individualized therapy plans based on patient goals and input from families and physicians.
  2. Deliver skilled therapy services directly to patients.
  3. Adjust therapy plans based on patient progress.
  4. Train caregivers on home exercises and strategies to support therapy.
  5. Work as part of an interdisciplinary team using HomeHealthMD for coordination.
Documentation & Compliance
  1. Complete necessary documentation like evaluations and progress notes within agency and regulatory timelines (48 hours).
  2. Accurately record therapy contacts, cancellations, and outcomes.
  3. Document therapy elements for proper billing and compliance.
  4. Communicate any changes regarding patient status to the care team.
Communication & Collaboration
  1. Participate in interdisciplinary meetings and case discussions.
  2. Share findings and recommendations with physicians, nursing staff, and leadership.
  3. Maintain open and respectful communication with patients and families.
Additional Duties
  1. Potential for on-call therapy coverage as needed.
  2. Support scheduling and resource management.
  3. Engage in continuing education to meet licensure requirements.
Requirements:
  1. Valid Speech-Language Pathologist license in California.
  2. Preferred minimum of 1 year experience in home health, outpatient, or related settings.
  3. Certification with ASHA (CCC-SLP) or equivalent credential.
  4. Current CPR / BLS certification.
  5. Reliable vehicle, valid driver’s license, and insurance.
  6. Strong communication and organizational skills.
Work Environment:
  • Home visits throughout selected counties.
  • Flexibility in scheduling (full-time or part-time) with remote documentation available.
