Activa Home Health (an AccordCare Company), a licensed Medicare-certified home health agency, is seeking an experienced Per Diem Clinical Review Manager (CRM) to support our patients and clinicians. Requirements:
- Dual certification in OASIS and diagnosis coding (required)
- Proven experience in clinical review and home health documentation
- Ability to assist the team with overflow charts, especially during month-end and PTO coverage
- Strong skills in reviewing and completing 30-day summaries
- Experience with face-to-face review with coding and QA teams
- Knowledge of creating 485 forms for SOCs and updating recertification OASIS forms
- Ability to review post-facility discharges and create post-hospital orders
- Experience in reviewing and updating coding for OASIS follow-ups (SCIC)
- Strong communication and collaboration skills with field clinicians regarding OASIS documentation updates
- Ability to provide education and follow-up as needed to ensure accurate and compliant documentation
- Support team with overflow charts and coverage needs
- Conduct face-to-face reviews with coding and QA teams
- Create and update 485 forms for SOCs and recertifications
- Review post-facility discharges and create post-hospital orders
- Update coding for OASIS follow-ups (SCIC)
- Collaborate with field clinicians on documentation, provide education, and ensure follow-up
