At ViaQuest Psychiatric & Behavioral Solutions, our nurses who provide Health Services Coordination/Consulting for Clients with an Intellectual and Developmental Disability, provide nursing oversight of medication and appointments, education and support to clients, staff, providers, Franklin County, Ohio, and Client families/guardians through site visits, updated health histories, phone calls and computer assignments. Communicate with guardians, Service Coordinators, Physicians, and group home staff to help ensure the health and safety of the Client on the assigned caseload. Responsibilities may include:
  • Monthly Healthcare Services Coordination/Consulting Nurse Site Visits, MAR change over, review of Client medication and appointments, updating medical histories, team meetings, educating staff on new orders, communicating with members on the team to ensure everything is in compliance with DODD rules and regulations, QA audits of charts, and resource for staff to contact for medical questions and concerns.
  • Paid on-call rotation.
  • Maintaining current Client records and progress notes.
  • Maintaining regular communication with RN/Team Lead concerning Client and family care needs.
Requirements for this position include:
  • Graduate of an accredited nursing program.
  • RN or LPN license.
  • Nursing or Client care experience preferably within DD and Mental Health areas.
  • Valid driver's license, car insurance and reliable transportation.
  • Willingness to travel throughout assigned service area.
  • CPR or Course can be provided during training.
