Job Title: Registered Nurse (RN) Case Manager Home Health Specialist
Schedule: Full-Time (Monday to Friday, 8:00 AM 5:00 PM), 1 weekend/month, 1 on-call shift/week
Compensation: $85,000 $130,000 annually + $6,000 in bonuses during first year of employment

- Lead comprehensive nursing assessments and individualized care plans
- Conduct OASIS visits and coordinate services across care settings
- Collaborate with physicians and providers to maintain clinical continuity
- Deliver education, emotional support, and transition planning for families
- Ensure accurate documentation and regulatory compliance
- 3+ years of clinical RN experience; home health preferred
- Strong documentation, care planning, and assessment skills
- Reliable vehicle and valid drivers license
- MN RN license in good standing
