RN Case Manager Hospice Promise

Hospice Promise is a nurse?owned organization dedicated to individualized, holistic end?of?life care. We are looking for a compassionate RN Case Manager to coordinate care for patients in?home.

Key Details

Hours: Monday Friday, 8:00?am 5:00?pm

Driving: Must be able to drive to patients homes; personal vehicle and valid drivers license required.

Benefits: Medical, Dental, Vision, Life; 80 hours vacation per year; 40 hours AZ sick time.

Responsibilities
  • Complete initial, ongoing assessments of patient and family to determine hospice needs.
  • Provide comprehensive physical assessment and maintain patient medical history.
  • Utilize nursing process to deliver professional care.
  • Write/implement and revise individualized plans of care in collaboration with patients, families, and interdisciplinary teams.
  • Administer medications and treatments per physicians orders.
  • Provide health education and counseling to patients and families.
  • Assist with activities of daily living, promoting self?sufficiency and comfort care.
  • Act as Case Manager for assigned caseload, coordinating care and overseeing hospice aides.
Qualifications
  • Registered Nurse (RN) with current licensure in Arizona.
  • Bachelors degree in nursing or equivalent; 12 years recent acute care experience, 12 years home care experience, preferred 1 year hospice experience.
  • CPR certification.
  • Strong communication, observation, problem?solving, and basic math skills.
  • Physical ability to walk, stand, lift, and transfer patients; good vision and hearing.
  • Personal vehicle and auto liability insurance.

Position is entry level and part?time.
