LCSW/ MSW needed asapCall Requirements: All work every third weekendFloating policy: All Social workers float-except trauma.Must have acute care adult hospital case management MIDAS/Meditech Experience, PASRR, Navihealth MSW or LCSW1 year experienceFL lic requiredPatient Experience requirements: Knowledge of Hospital DC planning/execution and knowledge of MDR or IDTDocumentation System: MIDAS/Meditech, Nate, Navihealth, Care Team AssignmentList Typical Procedures Performed on Unit(s): Discharge planning, MDR, Referrals, Working with Family and Patients
