This full-time, day-shift position is focused on delivering exceptional, patient-centered care to individuals and families facing advanced illness in a supportive, interdisciplinary setting.
Position Details
- Full-time, day-shift employment
- Provide comprehensive hospice and palliative care services
- Collaborate with physicians, nurses, social workers, and spiritual care providers
- Conduct patient assessments and develop individualized care plans
- Manage symptoms such as pain, anxiety, and shortness of breath
- Communicate effectively with patients and families regarding prognosis, treatment options, and end-of-life decisions
- Ensure timely and accurate documentation in compliance with regulatory standards
- Licensed Nurse Practitioner in the state of Illinois
- Certification in Hospice and Palliative Nursing preferred
- Strong communication, empathy, and clinical decision-making skills
- Ability to work both independently and collaboratively within an interdisciplinary care team

- Prime Healthcare Illinois Medical Group offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. Our Total Rewards package includes, but is not limited to, paid time off, a 401K retirement plan, medical, dental, and vision coverage, tuition reimbursement, and many more voluntary benefit options. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $120,000-$125,000 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
