Summary: The Arc of Union County Inc. is a leading provider of Person Centered services to individuals with intellectual and developmental disabilities through Adult Day Services programming. Reporting to the Assistant Director, Day Health Services and operating under the supervision of Adult Health Registered Nurse, Supervisor, Day Health Services (LPN) is responsible to assure the delivery of quality person centered services to all assigned service recipients in accordance with the Agency's goals of "Person Centered Planning" and within the guidelines of all Agency and DDD policies and procedures. The incumbent will maintain ongoing monitoring and documentation of residents' health needs, identify emerging health needs and provide appropriate guidance and intervention as directed. The incumbent is a member of an interdisciplinary team monitoring individual support needs and making recommendations relative to the identified needs.

Day Health Services programming provides education and training to acquire the skills and experience needed to participate in the community, consistent with the participant's Service Plan. This includes activities supporting participants with building problem-solving skills, self-help, social skills, adaptive skills, daily living skills, and leisure skills. Activities and environments are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence and personal choice. Adult Health Day Habilitation programming engages service recipients in small group outings to the community, leisure, educational activities and community experiences.

The Supervisor, Day Health Services will assure a high level of quality program supervision and management of the assigned program. The incumbent will assemble staffing team and directly supervise Day Health Services and other agency personnel as assigned or required. This position is responsible for staff evaluations and personnel recommendations. The incumbent will assemble of a staff team and assure the development of staff competencies. Foster and maintain positive employee/management relations; develop an atmosphere of cooperation and teamwork.

Supervisor, Day Health Services (LPN) ensures provision of services in a safe, secure, and dependable support and assistance in areas that are necessary for service recipients to achieve full social inclusion, independence, personal and economic well-being as approved by the Individual Service Plan (ISP). Supervisor, Adult Health Day Habilitation facilitates and ensures full social inclusion, independence, personal and economic well-being of service recipients. The incumbent ensures service recipients are progressing towards his/her personally defined outcomes as indicated in the Individual Service Plan (ISP).

This position requires thorough knowledge of Fee for Service model system. The incumbent is responsible for building a staffing team and supervision of Support Professional, Day Health Services, assigned to their program(s), the evaluation of staff and the recommendation and execution of personnel actions when necessary. The incumbent will assure Support Professional, Day Health Services, staff development as well as assuring staff compliance with agency, DDD, DHS Certification standards, CARF, and other applicable regulations, policies and procedures. The Supervisor, Adult Health Day Habilitation will maintain the program in acceptable practice and appropriate condition at all times for 3 Year Certification (Day Habilitation Certification), City Fire Safety Certification and full accreditation by CARF.

The incumbent considers the unique characteristics and needs of the individual as expressed by the individual and others who know the person, such as family, friends, service providers, etc., assures the service recipient is included in problem-solving and decision-making, and ensures services are provided in a non-intrusive manner.

The incumbent maintains a positive, healthy and stimulating environment for service recipients and assures adequate supervision at all times.

This position requires completion of DDD System Mandatory Training Bundle within 90 days of hire, DDD and Agency Orientations, other designated trainings and a minimum of 12 Hours of Professional Development completed annually; completion of New Required State DDD Education In-services. Maintain required New Jersey LPN certification, etc.

This is an exempt position.

This position requires that information related to service recipient, employee, program operations, agency is handled, channeled, managed and delivered professionally to the appropriate and right personnel or party consistent with agency and department policy. Any misuse, mismanagement, etc. of information obtained, accessed, discussed, or provided to position may result in removal from managerial role or position capacity.

Skills: This position requires Ability to use Windows software, Ability to use a data entry software such as Therap, Ability to complete Shift and Individual Served ISP Data entry in web based electronic platform. Able to demonstrate active listening. Excellent interpersonal and conceptual skills required. Ability to practice evidence based clinical practices within the scope of practice and/or as directed. Ability to demonstrate compassion, excellent communication skills, compliance with professional ethical conduct, demonstration of critical thinking, attention to detail, confidence, adaptability, stamina and commitment to development. Clear understanding of the Nursing Process and its application in an interdisciplinary community environment to enhance the lives of individuals with intellectual disabilities, mental illness and associated multimorbidities. Proficiency in all areas of service listed above. Strong management, leadership, planning and communication skills required. Demonstrate ability to manage information effectively. Ability to motivate staff, take responsibility for supervisory actions and establish quality controls. Strong conceptual and computer skills required. Must be able to read, write and speak English. Must have basic computer math skills.

Qualifications: Current NJ Licensed Practical Nurse Licensure required. One year supervisor level experience that includes three years combined Direct Support Professional and Assistant Supervisor role experience working with people with Individuals with Developmental Disabilities at front line management; and knowledge in the design and supervision of a program serving individuals with I/DD, and development of staff. Bachelor's degree required; degree in Social Services or supporting industry field of study preferred. Demonstration of a thorough understanding of knowledge, abilities and skills outlined above. Flexibility and demonstrated ability to thrive and perform in an ever-changing, busy and fast paced environment. Must be 18 years of age or older. Must have a valid New Jersey driver's license and abstract (not to exceed 5 points), and ability to meet agency insurability requirements. Complete State/Federal Criminal Background checks and Central Registry checks.

Major Position Responsibilities

Ensure all required information and documentations for service recipient's placement are obtained and maintained in service recipient's record file for effective plan development, effective service delivery, acquisition of a 3Year Day Habilitation Certification, fulfillment of Center for Medicaid Services requirements and agency objectives.

Ensure program operational practices are aligned with DDD Day Habilitation Standards, Supports Program Standards, the agency Operational Standards etc. Ensure the program is fully prepared for a successful day habilitation inspection/audit and attainment of a 3 Year Day Habilitation Certification.

Ensure the transport of service recipients to/from program and to the community for outings by program staff.

Ensure staff inspect the vehicle thoroughly before and after each trip. Verify that the assigned vehicle and equipment is in a manner that provides safety and comfort to all individuals served and staff members by checking vehicle safety such as tires, brakes, turn signals, mechanical equipment, etc.

Inspect and assure that that the safety monitoring devices in the vehicle are intact as installed, activated and always operating as intended, prompt follow through on malfunctions, assure free of interference and prompt notification with immediate supervisor on any deviations.

Ensure that staff complete re-trip and post-trip inspection and vehicle mileage form etc. of each vehicle assigned each time an agency vehicle is used and submit the forms completed periodically as required.

Maintain the vehicle in clean and safe condition inside and out and immediately report any vehicle mechanical or other problem to the coordinator, supervisor, or director.

Communicate to Transportation Department maintenance all vehicle fluids, oil, water, brake and transmission fluid at appropriate levels to insure proper vehicle operation.

Facilitate transportation runs with or without a driver aide as assigned by the transportation management.

Inform the guardians, family, providers in case there is about staff changes.

Visit transporting routes, drop off, and pick up sites regularly to assess, evaluate and review safe transporting activities. Receive front line staff input/feedback, interact with staff as necessary and make observation on service delivery relative to operational objectives.

Monitor overall health status of service recipients within the scope of practice and/or as directed including but not limited to regular weighing, blood pressure monitoring, blood sugar monitoring, etc., and make appropriate recommendations to enhance the service recipients' care.

Maintain proper knowledge of service recipients' health needs.

Perform body checks as required.

Monitor health related practices and medical procedures as indicated within the scope of practice.

Interpret medical reports under the guidance of Registered Nurses.

Collaborate with the team to ensure proper documentation, maintenance of service recipients' medical records and disseminate medical information as instructed, including training as needed to other staff, outside personnel, etc., within the scope of practice.

Accompany service recipients to medical evaluations as directed.

Provide skilled nursing care/treatment to service recipients as directed and within the scope of practice, e.g., wound care, ileostomy, colostomy care, other special procedures, etc. Medication administration may be required.

Serve as nursing liaison as needed with family members, professionals and other individuals/organizations in the community when indicated and within the scope of practice.

Assist with preparing the Program for inspections (i.e.: review of medical charts, prescription labels for compliance, menus for nutritional information, audit first aid kits, emergency numbers posted, assure medications are secured properly, etc.)

Participate as interdisciplinary team member in the development of ISPs. Attend and participate in all relevant meetings as directed.

Perform other duties as required by the nature of the position or as requested by Supervisor within the scope of practice. The duties must be performed consistent with Agency policies and New Jersey Nurse Practice Acts.

Ensure that all required information for service recipient's placement and service delivery is obtained.

Implementing the strategies necessary to assist the individual in progressing toward his/her personally defined outcomes as indicated in the ISP.

Run Therap activity report, audit and verify that self and each staff logs into Therap Daily and reviews all logs, Email, & S.Comm as required, each staff rotates daily in attending to or supporting different individuals in program, each staff rotates in various shift tasks namely: Rotation in community activities facilitation, Rotation in Med Administration & Meal prep, Rotation in Hygiene Support & Life Skills Training, Rotation in program cleaning, Rotation in Therap Data entry, Rotation in checking off on shift tasks as designated in Therap etc.

To ensure that the agency is able to bill Medicaid for the services provided, run Therap ISP Data report daily to assure every shift has inputted data daily (Daily Tracking) on Therap for the service (s) provided to the service recipient directly i.e. personal care goals, ISP/IHP goals. Assure immediate correction of any missed entries.

Develop and implement a weekly shift assignment schedule outlining daily staff rotations in assignments.

Ensure recording of data (in Therap) relative to assigned outcomes for service recipients including ISP tracking, Daily Training Record (DTR), etc.

Ensure completion and timely submission of monthly reports for service recipients including ISP reporting, service recipients' finances, agency petty cash, etc.

Document and communicate to support team, residential services, and other providers etc., any changes on the service recipient needs or functional abilities. Intervene as directed within the scope of practice.

Plan, support and assist service recipients in educational, enrichment or recreational activities as outlined in his/her Service Plan to enhance inclusion in the community.

Plan and assure service recipients healthful indoor and small group outings to community events, leisure and educational activities in the community.

Plan and assure that each service recipients participates in out of program community outing and/or experiences at least three times a week.

Ensure programming; provide assistance with activities of daily living, increase community participation, independence, safety awareness and learning activities.

As part of the direct support-staffing ratio, spend time on the activity floor, lead in facilitating programming activities, and provide self in supporting and relieving staff during shift sessions.

Implement agency policy that prevents assigning staff from other programs or rooms to accompany/transport service recipients from secondary programs or rooms (i.e., not their primary program/room) on community outings or trips. This measure ensures that staff are familiar with how to support the service recipient's needs and prevents confusion during transitions, which could otherwise result in the service recipient being left unsupervised in the community.

Ensure implementation of skill development, Behavior plans, ISP and other plans as developed and directed.

Recognize and respect individual served' rights, encourage independence, recognize and value competence and dignity, respect cultural/religious needs and preferences, promote employment and social inclusion, preserve integrity, support strengths, maintain the quality of life, enhance all domains/areas of development, promote safety and economic security.

Provide education and training to acquire the skills and experience needed to participate in the community, consistent with the participant's Service Plan. This may include activities t o support participants with building problem-solving skills, self-help, social skills, adaptive skills, daily living skills, and leisure skills. Activities and environments are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence and personal choice.

Promote individual interactions and experiences by emphasizing on age-appropriate community experiences, and offering a variety of activities and choices.

Develop positive, cooperative relationships with service recipients, families, guardians, support staff, volunteers, all agency departments/functional components, and community members including, CARF, DDD, and other State/Federal and Local agencies.

Prepare and submit donation requests annually on behalf of service recipients for invitation to Courier News Wish Book, etc., for the benefit of individuals served.

Maintain timely, thorough and effective communications to all appropriate parties regarding service recipients and operational service to program staff, support team and Assistant Director, Day Health Services Program.

Assure timely, thorough and effective communications regarding program, employee operations, to all appropriate parties including but not limited to program staff, supervisor, director and/or executive designee.

Responsible for ensuring that, upon notification of qualifying or active incident, appropriate measures are taken consistently with agency policies, procedures and training, and support is directed toward the staff and the service recipient involved in a crisis. Provide guidance, direction, intervention, etc. and ensure that incidents meeting the criteria of Unusual Incident Report are reported through automated UIR/GER documentation, standardized notifications activated as indicated through DDD circular and Arc of Union County trainings.

Responsible for ensuring that upon notification of an employee accident at work, appropriate measures is taken consistently with agency policies and procedures. Provide guidance, direction, and assure claim is reported through standardized notification including Employee Accident Report, Root Cause Analysis documentation.

Manage staff schedules and staffing coverage consistent with attendance capacity and budgetary dynamics.

Ensure sufficient staffing coverage for all shifts, utilize creative methods to address emerging staffing needs including tapping agency substitute pools through the electronic Dial My Calls System, consult colleagues for temporary staff rotation support, provide self as readily backup staff.

Monitor shifts to assure sufficient coverage as intended, assure staff's awareness of communication tools (Staff Roster Line Management Contact, Emergency" On Call Management Team/Administration etc.) at disposal/prompt communication to management on arising staffing situations for timely intervention.

Channel administrative, Operations or personnel matters within respective department's Line Management Team.

Manage staff schedules and staffing coverage consistent with attendance capacity and budgetary dynamics.

Review staff time sheets and monitor staff compliance with assigned schedule, time keeping policies and agency clock in/out procedures.

Follow assigned schedule and agency clock in/out procedures.

Maintain regular, reliable, and non-disruptive attendance is an essential job duty. Lack of adherence to assigned work schedule, abrupt call outs, patterned absences etc. undermines agency's obligation to provide consistent staffing, safe care and exerts hardship in the smooth operation of the program.

Develop a working knowledge of Day Habilitation Certification, CARF regulations and other regulatory bodies.

Demonstrate a thorough understanding of the Departmental Tactical Plan, Departmental Operational Plan, Agency Strategic Plan, and Human Resources Policies.

Promote dignity, self determination, social integration and service recipients' development opportunities. Foster a Person Centered Therapeutic Environment that encourages residents input and choices; Promoting programming that addresses recreational, social, vocational, health, safety and daily living activities of all service recipients.

Assure that the program complies with OSHA practices and has adequate working tools including Personal Protective Equipment, cleaning supplies, etc.

Facilitate employee Program Specific Orientation and ongoing training on agency policies and procedure. Secure the signed orientation documentation, file and distribute as required. Responsible for auditing training records regularly, register staff for training and assure that all staff members are fully trained in all DDD trainings meet criteria of a minimum of 12 Hours of Professional Development completed annually, State trainings, etc. assure proper re-certifications, monitoring attendance and addressing training attendance performance in a timely manner.

Ensure that staff members are fully trained on program specific trainings, department, agency and DDD trainings, and meet criteria of a minimum of 12 Hours of Professional Development completed annually, etc. Orient and train all assigned staff in Program specific and agency policies and procedures; monitor, and evaluate staff performance in trainings and compliance with all policies and procedures.

Teaches DSPs and self in knowing each service recipient's Health Baseline, monitors changes, intervenes promptly as trained and consistent with agency policy to assure that the service recipients' health does not deteriorate to the extent of undermining their service health and safety

Complete all supervisory trainings as designated and attend ongoing professional development management workshops as directed.

Respect and maintain confidentiality of all information pertaining to service recipients, employees and others throughout the Agency.

Assure each service recipient has an Individualized Service Plan (ISP) developed.

Review provided information related to the individual, including the PCPT, previous ISP(s), available assessments, and evaluation data, as appropriate/relevant.

Participate actively in the planning of team meetings and working cooperatively to achieve consensus in the spirit of the ISP operating principles. Participate in the IDT, ISP process and assure identified goals are captured and accomplished in accordance with Agency policies. Ensure correct implementation of ISP objectives and Behavior Plans as written. Attend all service and professional meetings pertaining to the quality of life of the service recipients and the service delivery systems of the program.

Ensure data relative to assigned outcomes are recorded as stipulated.

Notify the Support Team and request a special team meeting to be scheduled whenever there is a significant change in the individual's status.

Responsible for completing and/or updating service recipients' Evacuation Evaluations with the Assistant Director, Day Health Services Program.

Utilize agency approved Medication Administration Records (MARs), compare with previous month's MARs, compare MAR to prescription, Bubble pack, etc., for accuracy.

Assure Agency policies on medication administration and documentation are followed thoroughly and consistently as directed.

Assure adequate medication refills, adequate medication supplies at all times and free of medication administration errors.

Assure staff members are writing the date, the time and their initials on the bubble pack/bingo cards after administering medication.

Conduct meticulous random medication pack/bingo cards audits to ascertain proper medication administration and charting.

Ensure completion of all required documentation/records and administrative assignments relative to the program, service recipients, staff and agency administration in accordance with agency standards, DDD and other applicable requirements.

Responsible for organizing and leading staff meetings and assure the completion of staff meeting minutes.

Maintain positive employee/management relations; develop an atmosphere, which fosters employee morale and growth.

Effectively develop staff through training, leadership, role modeling, constructive feedback, praise and progressive discipline.

Effectively orient and develop Support Professional, Day Health Services to function professionally and deliver high quality services.

Develop, maintain and account for all financial records and reports for program and service recipients' finances. Ensure effective management of agency petty cash and service recipients' funds, proper documentations appropriate practices consistent with agency policy. Submit and distribute financial reports as directed.

Organize program and ensure operation is consistent with Agency and department operational systems as indicated.

Assure program expenditures are consistent and within budgetary guidelines.

Maintain required Day Habilitation certifications, standards and manuals. Materials are availed to staff and management as needed.

Effectively Partner with Agency Registered Nurses, QA, Behavioral, Maintenance, Transportation etc. to enhance the quality of services and meet Program, Department and Agency goals and objectives.

Collaborate with Assistant Director, Day Health Services to effectively and efficiently interview and hire qualified applicants.

Responsible to assuring the program and facility grounds are maintained in appropriate condition and the environment is clean, safe, neat, and organized.

When necessary, serve as backup or replacement for Support Professional, Day Health Services, to ensure proper coverage and care for service recipients.

Inform Assistant Director, Day Health Services in a timely manner about guardian's refusal to comply with agency service delivery policies/procedures, provision of required documents, or declination to consent to service delivery, e.g. medical treatment, community integration activities, photograph taking/publication, etc.

Initiate "on call" procedures with respect to arising incidents including 911 emergency calls, life threatening incidents, and/or operational breakdown etc. and complete Unusual Incident Reports (UIR) in a timely manner consistent with agency policy and state law.

Responsible to report to immediate Supervisor or through the chain of command any suspected incidents of abuse, neglect or exploitation of service recipients and complete Unusual Incident Reports (UIR) in a timely manner consistent with agency policy and state law.

Monitor and safeguard agency property and funds.

Promote the health and well-being of individuals served, monitor service recipients' wellbeing, report promptly any health changes consistent with agency protocols, transport service recipient to doctors office for treatment and/or accompany individuals served to Emergency Room as needed.

Perform duties according to the methods, manner and spirit of the ideals of the field of Human Services and within the profession/job specific guidelines, including wearing appropriate attire etc.

Downloading, copying, transferring, and transmitting of agency documents to personal accounts, and/or distribution externally is prohibited. The agency uses various IT systems to detect these activities.

Must fully cooperate with all investigations, inspections, etc. internally and externally including full cooperation with the Department of Human Services, the Division of Developmental Disabilities, Office of Licensing, Licensee or any other duly authorized person regarding the care and service given to individuals served by this Agency.

Employees who may encounter individuals served shall be subject to and comply with the requirements for random drug testing for controlled dangerous substances. Criminal history record background checks will be conducted at least once every two years for agency's employees. Any employee who is required to undergo a criminal history record background check and refuses to consent to, or cooperate in, the securing of a criminal history record background check shall be immediately removed from the person's position and the person's employment shall be terminated.

The Arc of Union County, Inc. is contracted with the New Jersey Department of Human Services to provide and facilitate essential personal care and health promotion services to individuals with disabilities and other comorbid conditions. Physical Demands: The physical demands for this position must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee may be regularly required to maintain a stationary position (standing and sitting), move/traverse, kneel, bend, reach with hands and arms for extended periods of time, twist, carry, lift up to 35 pounds, squat and drive a 6-person passenger van, may require operating manual devices such as hydraulic lifts, wheelchair lifts, vehicle doors, etc. for the purpose of transporting service recipients. Agility with wrist and finger dexterity to access, key and sort electronic information or file documents, ability to view computer and/or electronic device for extended period and other physical duties as directed by your supervisor and/or Director.

Ability to apply Agency Training Crisis Management Verbal and Physical Techniques during a crisis consistent with criteria as trained and in accordance with agency policies and procedure.

The essential Cognitive Functions of this position may include ability to analyze, compute, hear and repeat information, take notes, read documents, generate written information, independently problem solve, tolerate interruptions, maintain positive public relations, organize, verbally communicate and converse with a diverse group of people among others.
