Physical Therapist Assistant (PTA) at Pleasant Hill Post Acute

Pleasant Hill Post Acute is conveniently located off Pleasant Valley Drive and Via Del Sol in Pleasant Hill, California. We are currently seeking a dedicated Physical Therapist Assistant (PTA) to join our team.

Job Description:
  • Treat patients as directed by the Physical Therapist.
  • Record daily treatment notes and weekly progress notes in accordance with PT Board guidelines.
  • Assist in maintaining the department's functionality.
  • Participate in patient care and rehabilitation conferences as needed.
  • Help with the cleaning and maintenance of the treatment area.
  • Implement treatment plans as prescribed by the physician.
  • Assist the nursing department with training Restorative Aides.
  • Communicate with the supervisor and other health team members regarding patient progress, issues, and plans.
  • Participate in in-service training programs for other staff members.
  • Record any changes to treatment as per policy and procedures.
  • Instruct patients' families or nursing staff on maintenance programs and caregiver training in preparation for discharge from therapy services.
  • Report any issues with departmental equipment to ensure it is properly maintained.
Qualifications:
  • Must possess a valid Physical Therapist Assistant license.
