With 40+ years of experience in healthcare staffing, SHC connects clinicians with top home health partners nationwide.Qualifications: Active, unrestricted Physical Therapist (PT) license in the state of assignmentCurrent BLS certification (AHA)1+ year of recent home health experience (within the last 2 years)Experience conducting PT evaluations and developing therapy plansFamiliar with Medicare-certified home health guidelines and OASIS-E documentationExperience with EMRs such as Homecare Homebase, NetSmart, or EpicClear background check, driving record, and up-to-date health clearance (TB, immunizations)Reliable transportation with valid auto insurance and ability to travel between patient homesHome Health Physical Therapist Contract Details: $2,138 - $2,333 per week13-week contractPaid by the hour - not by the visit5–6 visits per day (45–90 minutes each, based on complexity)Travel time factored into productivity expectationsKey Responsibilities: Perform initial and ongoing PT assessments and develop individualized plans of careProvide skilled interventions for strengthening, endurance, pain management, and fall preventionConduct gait and balance evaluations, home safety assessments, and mobility trainingEducate patients and caregivers on exercise programs, adaptive techniques, and safety strategiesCollaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary careDocument all care in compliance with Medicare, OASIS-E, and agency standardsApply today to bring your skills to home health and help patients thrive where they’re most comfortable – at home.What We Offer: Full medical, dental, vision, life, and even pet insuranceRound the clock support.
