Job Details
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Valid state licensure as a Physical Therapist, minimum of 2 years of experience in a Skilled Nursing Facility setting, and proficiency in PointClickCare EMR.
- Schedule: Day shift, 8 hours per day, minimum 40 hours per week.
- Responsibilities include developing and implementing individualized treatment plans, documenting patient progress, and collaborating with a multidisciplinary team.
- Preferred Qualifications: Experience with geriatric populations and familiarity with SNF protocols.
