Responsibilities Home health setting with a post-acute care focusPatient population includes orthopedic/joint replacement and geriatric patients with multiple comorbidities such as CHF and COPDAverage caseload is 5 home visits per dayPT staff perform start-of-care visits, especially with orthopedic patients when nursing is not orderedUses Cerner charting system and Netsmart EMR; a laptop will be provided for EMR documentationWork schedule consists of 8-hour day shifts with occasional Saturdays for orthopedic patientsNo call or holiday shifts requiredTeam includes an intake RN, manager, and nurse aidesMain responsibilities include managing a caseload of home health patients and performing both routine and start-of-care visitsAverage daily census is 78 and is expected to increase with more therapy staffScrub color is not specifiedHoliday coverage is not requiredNo ventilator or call responsibilitiesReturning clinicians cannot have been employed by the related health system within the past yearOrientation consists of 12 hours and is floor-specificCandidates living within specific mile radii in certain states may have location requirements for eligibility Requirements Minimum 2 years of recent home health physical therapy experience requiredActive state or compact (NLC) license required if applicable; pending licenses not acceptedCurrent BLS certification requiredAbility to drive approximately 40 miles per day within the service areaMust provide a copy of driver’s license for considerationTwo professional references required, including at least one supervisor reference from the last 12 months and one peer or supervisor from the last 3 years which must state dates of employment and rehire eligibility Benefits High Pay and BonusesMedical, Dental and Vision Insurance with Day 1 Options401(k) PlanWeekly Payroll DepositFree Online CEUsGenerous Housing AllowanceTravel and Licensure ReimbursementsNon-taxed Per Diem and SubsidyTraveler Rewards and Discounts American Traveler Job ID #P-648951.
