Glimpse Edge Healthcare, Inc., a leading provider of healthcare services, is seeking a dedicated and experienced Nurse Practitioner to join our team in a specialized cardiology practice. This full-time position will be based at our assisted living facility located in Spring Valley, New York. Work Environment
This role is situated within an assisted living facility, offering a dynamic and collaborative environment where you will work closely with a team of healthcare professionals, including cardiologists and other nurse practitioners. The facility is designed to support the health needs of seniors, ensuring a compassionate and holistic approach to patient care. Initially, you will work five 8-hour shifts during the three-month training period, with a transition to four 10-hour shifts to foster work-life balance.
Key Responsibilities
- Conduct comprehensive health assessments by obtaining detailed medical histories and performing physical examinations to identify patient health needs.
- Order, perform, and interpret diagnostic tests such as EKGs, stress tests, and lab tests, ensuring thorough evaluations of cardiovascular health.
- Develop and implement individualized treatment plans including both pharmacological and non-pharmacological options, coordinating care with supervising cardiologists.
- Document patient progress meticulously in electronic health records (EHR), including writing orders and progress notes that accurately reflect changes in patient status and treatment interventions.
- Engage in patient education, providing clear communication about treatment plans, medications, and lifestyle modifications that enhance cardiovascular health.
- Provide regular follow-up care and health maintenance for patients, adhering to protocols established in collaboration with supervising physicians.
- Actively participate in team meetings and consult with interdisciplinary teams to optimize patient care and outcomes.
Patient Population / Client Focus
The ideal candidate will work with a diverse population of elderly residents who often present with complex medical histories and multiple comorbid conditions. Your role will be crucial in ensuring effective management of chronic cardiovascular issues, while also promoting overall health and functional independence among seniors.
Qualifications
- A Master’s degree in Nursing from an accredited institution.
- Current and valid Registered Nurse (RN) license in the State of New York.
- A minimum of 3 years of clinical experience in a relevant setting, preferably within cardiology or primary care.
- Basic Life Support (BLS) Certification to ensure readiness for clinical emergencies.
- Exceptional interpersonal and communication skills, with a strong capacity to work collaboratively with a diverse patient population and healthcare team.
Preferred Experience
While a minimum of 3 years of clinical experience is required, candidates with additional experience in geriatrics or cardiology will be highly valued. Familiarity with EHR systems is preferred, as well as experience working in assisted living or similar environments.
Compensation and Benefits
