About the Facility - Santa Fe Community Based Outpatient Clinic (CBOC)
This outpatient clinic provides care which includes Behavioral Health Care, Primary Care, Occupational Therapy, and Personal Care. They also provide Veterans with Telehealth, for access to health care that is timely, more convenient and easier to access through the use of new health care technology including video conferencing and health monitoring devices that can connect patients to their healthcare team. In addition to their main facility in Albuquerque, they offer services in 13 community-based clinics (CBOCS, such as this facility). Sister CBOC facilities are located in: Alamogordo, Artesia, Durango, Espanola, Farmington, Gallup, Las Vegas, Rio Rancho, Raton, Santa Fe, Silver City, Taos and Consequences.

RESPONSIBILITIES (listed are included but not limited to):
Nurse Practitioner's Responsibilities -
  • Will be responsible and accountable for assessment, diagnosis, and management of patient problems, including prescription of pharmacologic and non-pharmacologic intervention.
  • The scope of practice of a nurse practitioner includes, but is not limited to, each of the following:
    • Advanced practice registered nursing by certified nurse practitioners is based on knowledge and skills acquired throughout an advanced practice registered nurse's nursing educations, training, and experience.
    • Advanced nursing patient assessment and diagnosis.
    • Ordering diagnostic and therapeutic tests and procedures, performing those tests and procedures when using health care equipment, and interpreting and using the results of diagnostic and therapeutic tests and procedures ordered by the advanced practice registered nurse or another health care professional.
    • Ordering treatments, ordering, or applying appropriate medical devices, and using nursing, medical, therapeutic, and corrective measures to treat illness and improve health status.
    • Providing palliative and end-of-life care.
    • Providing advanced counseling, patient education, health education, and patient advocacy.
Physician's Assistant's Responsibilities -
  • Will provide quality patient care in collaboration with other health care professionals.
  • Ability to utilize the principles of PA practice in the care of assigned patients to provide appropriate care.
  • Ability to formulate a plan and provide treatment to the patient.
  • Ability to provide patient care that is abreast of changing concepts and advancements in the profession.
  • Ability to function autonomously and make independent medical decisions.
  • Ability to work effectively in an interdisciplinary environment for improvement of services for patient care.
  • Will function as experts in clinical practice and/or in areas related to the assigned roles and responsibilities, may serve as mentors for transitioning PA's and may provider training for health care students.
  • Skilled to consistently demonstrate an elevated level of expertise and autonomy in diagnosing and treating seriously ill, multi-symptomatic patients through facility Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE).
  • Ability to present clinical findings and recommendations to appropriate management officials.
  • Ability to clinically facilitate the delivery of patient care in collaborating with health professionals, health managers, and other health care personnel.
  • Ability to implement and initiate clinical changes to established treatment protocols based upon new medical literature research findings to enhance patient delivery care for improved patient outcomes.
REQUIREMENTS:
Nurse Practitioner's Requirements -
  • Must have a Master's degree in nursing.
  • Must have a current and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • Must have a current Basic Life Support and Advanced Cardiac Life Support certification by a provider recognized by the American Heart Association.
  • Must have a minimum of one-year experience in the clinical areas of intended assignment.
  • Must have full practice authority license in New Mexico.
Physician's Assistant's Requirements -
  • Graduate of an accredited Bachelor's or Master's program in physician studies.
  • Current and continuous certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Licensure: PA's must possess an active, current, full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
