JOB TITLE Peer Wellness Coach SUMMARY: Peer Wellness Coaches perform activities in alignment with the My Health, My Choice, My Life initiative in Tennessee. Peer Wellness Coaches lead wellness-based workshops, provide one-onone wellness coaching, and lead social/recreational activities to promote wellness among individuals with mental illness. Peer Wellness Coaches must identify as a person with lived experience who has met the criteria for a diagnosable mental illness, substance use disorder, or co-occurring disorder at some point in their adult life. Peer Wellness Coaches must also demonstrate a personal commitment to living a heathy lifestyle. EXPERIENCE: Education: Bachelor’s degree required. Master’s degree preferred. Certification: Upon hire, obtain Certified Peer Recovery Specialist (CPRS) certification from the State within required timeframe. Experience: Past/present utilization of mental health services required. Physical: Lifting up to 50 pounds. Pushing/pulling up to 150 pounds. Frequent sitting, standing, driving, walking, bending, stooping, and reaching. Knowledge/Skills: Possess excellent verbal/written communication skills. Ability to maintain composure in difficult situations. Ability to safely operate a motor vehicle. Must have intermediate skills in Microsoft Office programs. Must have intermediate skills in video communication software. EQUIPMENT: Computer, fax, copier, and any other equipment required to perform the functions of the position. MAJOR DUTIES AND RESPONSIBILITIES: 1. Implement the My Health, My Choice, My Life initiative to improve the physical health behaviors and physical health status of adult Tennesseans (18+ years of age) who live with mental illness, substance use disorders, or co-occurring disorders. 2. Complete all trainings/certifications/workshops made available. 3. Meets and complies with all State reporting requirements. 4. Prepare reports, presentations, and documentation in accordance with formats and timelines as directed by the State. 5. Convening and participating in scheduled meetings, health and wellness events, and conferences as designated by the State. 6. Co-facilitate My Health, My Choice, My Life trainings in accordance with grant requirements. 7. Adhere to the State Peer Recovery Specialist Code of Ethics. 8. Provide health and wellness programs in accordance with grant requirements. 9. Collaborate with agency staff of each location to coordinate health and wellness activities, including but not limited to, physical activities, healthy snacks, workshop graduation celebrations, brief interventions for Peer Wellness Coaching, other wellness activities, etc. 10. Log all health and wellness activities including but not limited to, physical activities, healthy snacks, workshop graduation celebrations, brief interventions of Peer Wellness Coaching, phone consultations with staff and clients, and other wellness activities, in a format prescribed by the State. 11. Design/supplement program curriculum with appropriate materials and guest presenters from local resources. 12. Develop liaison relationships with case managers and local health care providers and work with participants to establish an ongoing relationship with a primary health care practitioner. 13. Provide support to participants engaging with primary care physicians as needed. 14. Administer the workshop self-survey to each workshop participant at the end of each workshop. 15. Reports data from workshop self-surveys in the Monthly Reports to the State. 16. Administer the Goal Attainment Scale (GAS) to each Peer Wellness Coaching participant per grant requirements. Complete GAS Quarterly Summary and submit with the Monthly Report per grant requirements. 17. Submit quarterly reports by the fifteenth (15th) of October, January, April and July to the State in the format prescribed by the State. 18. Submit the final year report in a format and timeline prescribed by the State. 19. Assist the State, as necessary, with all initiative events and activities and participate in other activities as approved, directed, and authorized by the State. 20. All other duties as assigned. Demonstrate initiative and strive to continually improve processes and relationships. 11. Follow all Frontier Health rules, policies and procedures as well as any applicable laws and standards.
