Responsibilities THERAPIST- BEHAVIORAL HEALTH Horizon Health is seeking an OTP Counselor/ Therapist for our clinic in Highpoint, NC. The OTP Counselor/ Therapist is responsible for providing counseling, educations, and referral services to enrolled patients and their families as defined by OTP protocols. Counseling services include individual, group, and family counseling. RESPONSIBILITIES: Conduct Individual, group, and family therapy. Complete social services assessment on patients enrolled in the OTP Develop patient-specific short term goals and social services interventions and documents on treatment plan Complete social services updates to the treatment plan as assigned Conducts patient education programs according to needs identified, including health and wellness education Participates in treatment team meetings, providing feedback to the interdisciplinary team Make and maintain contacts with families, referral sources, and significant others as per policy.
