Every innovation, medical breakthrough, and groundbreaking treatment is powered by some of the best minds in medicine, ensuring nothing is left on the table or the road to recovery.A Brief OverviewThe Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately and that there is oversight of the multidisciplinary home care staff. Assists in the coordination of the clinical and leadership activities of the Agency. Collaborates with the Director of Home Care in developing, assessing, planning, implementing, directing, and evaluating patient care.What you will doSupervises Home Care Nurses, Physical Therapist, Occupational Therapists, Speech and Language Pathologists, and Registered Dieticians by reviewing and evaluating each case through a variety of means such as joint visits, case conferences, interdisciplinary conferences, handoff reports and record review (clinical & billing)Assists clinicians in identifying skills need, addressing social/safety concerns of patients, setting priorities and developing appropriate plans of careInstructs and guides clinicians to promote more effective performance and delivery of quality servicesAttends case conferences meetings with home care staff to facilitate coordination of care and discussion of interdisciplinary group involvement and patients’ progress towards goalsAvailable during operating hours to assist clinicians as appropriate and necessary as a clinical resource or conflict resolutionParticipate in Administrative On Call Rotation as scheduledActs as a liaison between staff, patients, organization & AgencyManages scheduling process to ensure compliance with utilization, supervisions and appropriateness of skillsCollaborates with the Intake Team to properly manage new referrals, review patient information related to referral and assign appropriately. Conferences with the Director regarding any questions about an individual's eligibility for services, while complying with regulatory requirementsParticipates in record reviews according to QAPI guidelines and communicates findings to Quality MembersIdentifies staff educational needs and functions as a clinical resource/role model for nursing personnelAssist in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assist in the orientation of new employeesAssists Director and other leadership staff in the planning, implementation and evaluation of in-serviceAssists Director and other leadership staff in the review and revision of policiesProvides supervision to direct reports on an ongoing basis to ensure complianceWorks collaboratively with the Director to assure documentation is complete, timely and consistent with careResponsible for day to day management of all clinical staff including annual performance reviews as required and any performance management issuesManages staff PTO requests, holiday assignments and daily calendarSeeks opportunities to further own knowledge in areas of responsibilityCompetent in organizational skills to facilitate the delivery of careDemonstrates a proactive attitude by anticipating potential problem situations and appropriately intervenesProvides for a positive environment for clinical staffDemonstrates effective communication with other departmentsResponds to identified concerns of patients, physicians/providersPerforms other related duties as assignedEducation QualificationsGraduate of an accredited school of nursing. required andBachelor's Degree BSN preferredExperience Qualifications1-3 years Relevant experience in Home Care, Hospice and/or Community Health Setting. requiredKnowledge, Skills, and AbilitiesStrong leadership and clinical skills.Excellent communication, both verbal and written as well as strong decision making, organizational and problem solving skills are neededLicenses and CertificationsRN - Registered Nurse - State Licensure and/or Compact State Licensure Current New Jersey Registered Nurse License upon hire required andDL NUMBER - Driver License, Valid and in State upon hire required andCPR - Cardiac Pulmonary Resuscitation upon hire requiredAt Holy Name, we believe in rewarding every team member with more than a paycheck—we invest in your future and well-being.
