The Orchards at Canterbury on the Lake - The Social Services Assistant provides administrative and clerical support to the Social Services Department while assisting with discharge planning processes. This position supports the coordination of resident services, documentation, and communication with interdisciplinary team members, residents, families, and external agencies to ensure safe and effective transitions of care. Essential Duties and Responsibilities Clerical and Administrative Support Maintain accurate and up-to-date resident social services records in accordance with facility policies and regulatory requirements Assist with data entry, filing, scanning, and record management (electronic and paper) Prepare correspondence, reports, forms, and discharge-related documentation Answer and direct phone calls; communicate professionally with residents, families, and outside agencies Schedule meetings, care conferences, and discharge-related appointments Track referrals, follow-ups, and required documentation deadlines     Discharge Planning Support   Assist the Social Services Director with discharge planning coordination Communicate with residents, families, case managers, home health agencies, and community resources regarding discharge needs Prepare and distribute discharge paperwork and instructions Assist in arranging post-discharge services such as home care, durable medical equipment, transportation, and community resources Document discharge planning activities in the medical record as directed Resident and Family Support Provide clerical assistance during resident admissions and discharges Serve as a point of contact for non-clinical questions related to social services Maintain confidentiality and professionalism in all resident interactions Compliance and Team Collaboration Adhere to HIPAA, state, and federal regulations Work collaboratively with the interdisciplinary care team Participate in department meetings, audits, and quality improvement activities as assigned Qualifications Education and Experience High school diploma or equivalent required Associate degree or coursework in social work, healthcare administration, or related field preferred Previous experience in a healthcare, skilled nursing, assisted living, or social services setting preferred Skills and Abilities Strong organizational and clerical skills Proficiency in Microsoft Office and electronic medical record systems Excellent written and verbal communication skills Ability to manage multiple tasks and prioritize effectively Compassionate, professional demeanor with residents and families Ability to maintain confidentiality and handle sensitive information Physical Requirements Ability to sit, stand, and walk for extended periods Ability to lift or carry up to [X] pounds as needed Ability to operate standard office equipment
