Working together with facility, health system, and network partners, Ultra Health helps speed recovery and lower rates of hospital readmission.Position OverviewWorking collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of residents within skilled nursing facilities. The APRN will build strong working relationships with administrators and staff throughout their designated building and serve as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what they loveworking independently to see patients and treat complex medical conditions.ResponsibilitiesProvide primary, urgent, and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents.Verify admission orders and conduct initial assessments of residents. Conduct physical exams, medication reviews, and depression screenings.Collaborate with the facility medical director, attending physicians, and nursing staff to deliver the highest level of care.Communicate with and counsel family members on treatment options when there are material changes in status.Minimum QualificationsValid Nurse Practitioner License in applicable stateDEA LicenseNurse Practitioner national certificationClinical background in adult, family, or geriatric specialtiesCOVID-19 vaccination required unless qualified for a remote, religious, or medical exemptionPreferred QualificationsExperience in internal medicine, gerontology, or cardiologyExcellent administrative, organizational, and communication skillsAbility to work independentlyDetail-oriented, dependable, and reliableNursing experience in LTC/SNF a plusBenefits:Paid Medical Malpractice Insurance401(k)401(k) matchingDental insuranceHealth Savings AccountHealth insurancePaid time offVision insuranceSupplemental Pay:Bonus opportunitiesMake a meaningful impact in the lives of seniors.
