DescriptionThe Clinical Program Manager will have clinical and administrative responsibilities to ensure that the Department of Neurology is run according to policies and regulatory requirements, and will work in conjunction with the Queens Stroke Program Medical Director and will directly report to the Director of Neurology. The manager will act as the Supervising Advanced Practice Provider, and directly oversee the Advanced Practice Providers in the Department of Neurology. ResponsibilitiesClinical ResponsibilitiesTo work as an Advanced Practice Provider in the Department of Neurology; duties include direct in-patient consultation services and outpatient care.Teaching and supervision of Advanced Practice Providers, residents and medical studentsWill be responsible to assess any process delays and work on PI projects as associated with the supervisory roleAdvocates for patients and family members, by designing policies and services that promote patient rights and optimal environments of care, building on customer satisfactionAssists with stroke code activations to ensure protocols are followedAssists in coordination of the tele-stroke program and transfers among facilitiesOperations/Administrative Responsibilities:To work with and support the Director of the Department of Neurology in establishing and maintaining patient care policies and procedures and ensuring the department conforms with the bylaws, rules and regulations of the NYC Health + Hospitals | Queens Medical Staff and appropriate Federal, State and Local laws and regulationsContinuously monitors operational efficiency of the neurological servicesPrepares all required reports and performs the requisite data reconciliationCollaborates with physician leaders, specialists, nursing, service line team members, and other disciplines on stroke program policies, procedures, and protocols to improve outcomes for stroke patients and enhance the healthcare environmentIdentification of stroke patients on a real-time basis, and entering all required data elements into the designated stroke center database and registry Provides coordination and administrative functions for the multidisciplinary Stroke Steering committee Responsible for participating in stroke program marketingTo represent the Director of the Department and to act in their stead in case of their unavailabilityEducation and Outreach:Participates in community and educational outreach activities to improve awareness of neurological emergenciesIdentifies and facilitates opportunities for community engagement for stroke patients and caregivers i.e. stroke support groups, community events, and health fairsQuality of Care/Regulatory:Utilizes quality improvement data to identify and implement evidence-based interventions that reflect current scientific knowledge of neurology; practice and promote continuity of careEnsures the implementation of standards of care according to Clinical Practice Guidelines.Assesses the need for, plans, and implements quality improvement programs driving neurology quality measuresParticipates in patient follow up and monitors patient satisfaction scoresParticipates in Joint Commission and State Stroke Center certification efforts focusing on compliance of standardsIncorporates current stroke AHA guidelines and evidence-based practice into their respected stroke standards of practiceReviews and analyzes data for trends, shares data with stakeholders, and utilizes data for performance improvement initiativesAbstracts and manages data for Get with the Guidelines and the Joint CommissionAdditional duties may be assigned by the Director as needed:Assigns and schedules clinical staffSupervises Advanced Practice Providers in areas of responsibility and professional development Monitors and appraises performanceRecommends termination, discipline and advancement based on performance evaluations. Participates as indicated in the disciplinary processInterviews Advanced Practice Provider candidates and makes recommendations for hiringPlays a leadership role in the training of new employees.
