Job Title: Social Worker Location: Camden, ME 04843 Employment Type: Contract Duration: 14 Weeks Schedule: Day Shift | Monday–Friday, 8:00 AM–4:00 PM Hours: 40 hours per week (guaranteed) Pay Rate: $35–$42/hour Weekly Compensation: $1,312.50–$1,575 Job Overview "Navitas Healthcare, LLC" is seeking a compassionate and experienced Social Worker for a full-time, 14-week contract assignment in Camden, Maine. This day-shift role offers a consistent weekday schedule and guaranteed 40 hours per week. The Social Worker will provide comprehensive psychosocial support, advocacy, and case management services to residents and their families within a healthcare setting. Key Responsibilities Conduct initial and ongoing psychosocial assessments to identify residents’ emotional, social, and environmental needs Collaborate with interdisciplinary care teams to develop and implement individualized care plans Coordinate complex discharge planning, including community referrals, home health services, and durable medical equipment Provide short-term counseling and crisis intervention to residents and families Advocate for resident rights and assist with advance directives and power of attorney matters Support residents and families with financial resource guidance, including Medicaid and insurance navigation Maintain accurate, timely documentation in compliance with state and federal regulations Qualifications Licensure: Active Social Work license in the State of Maine (LSW, LMSW, or LCSW required) Education: Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred Experience: 1–2 years of recent healthcare social work experience preferred (SNF, Long-Term Care, or Acute Care) Skills: Strong knowledge of discharge planning, Medicare/Medicaid, and community resources Ability to work independently in a fast-paced environment Excellent communication and documentation skills For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
