Position Summary Under the direction of the program management, provides skilled casework services to selected caseloads or clients with special problems such as health disability or those at risk of nursing home placement. Provides specialized casework services aimed at securing the client’s overall well-being and maximum degree of independent functioning. Education Bachelors in Social Work, Sociology, Psychology, Healthcare or related field - Required Experience 2 Years human services or related field - Required License & Certification Certification provided by the Department of Community Health is required to be completed within 60 days from the hire date. Core Job Functions Performs comprehensive assessments geared towards developing care plans. Performs quarterly reviews based on outcomes sought in care plans. Develops care plans based on level of need and adapts care plan in response to changes in an individual's circumstances or care plan variances. Demonstrates knowledge and skills necessary to identify each person's needs and provide care that contributes to outcomes sought by care plans. Understand the importance of client satisfaction and excellence. Rapport established with client/caregiver, physicians and providers to facilitate coordination of service. Manages provider requests for service changes based on the care plan and individual’s needs and updates authorization documentation timely and accurately. Performs monthly contacts, quarterly site visits and reviews to provide effective case management. Works with the client’s primary care and support providers to ensure care plan is administered and followed. Meets with supervisor at least monthly to review cases and the progress or problems associated with duties. Maintains knowledge of case management manuals and community resources. Compiles and submits reports as needed. Assists in development and revision of policies and procedures. Document all required and relevant case-related information timely and accurately. Provides timely and accurate documentation to providers to broker and manage services to prevent interruption of service(s).
