Home Instead Minneapolis is seeking a dedicated and passionate Home Care Nurse to join our team. The ideal candidate would bring their technical skill, knowledge and experience to our communities and enhance the lives of the clientele we serve.

This position is a supervisory position assisting in oversight and delivery of non-medical home care services to our clientele who desire to age in place at home.
Responsibilities:
Conduct in-home Client Assessments and develop individualized Care PlansSupervise ongoing care of ClientConduct Quality Assurance and ongoing Reassessment of clientele under the MDH Home Care Statutes and RegulationsManage Client concerns and educational needs pertaining to careProvide supervision and education to Home Health Aides as neededAssist in Compliance and Management of accurate client records.Strong written, communication and interpersonal skills.Requirements:
Valid Minnesota RN LicensureValid Driver License and vehicle - travel reimbursement for all travel needsExperience in Acute care, Home Health, Hospice or Home Care settings is desirableAbility to work independently and manage time effectivelyThis is a wonderful position offering part time employment, with flexibility for those seeking work life balance
