Responsibilities: Evaluate wounds and collaborate with physician to ensure optimum delivery of care for wound healing. Record, measure and document resident's wounds/wound progress. Initiate nursing care procedures in managing acute, chronic and/or traumatic wounds. Track and monitor (for infection and healing) trending and record skin conditions and wounds/wound management. Coordinate treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation, and outcomes for patients throughout the facility. Provide patient, family and staff education regarding wound management and therapies. Act as the resource person for physicians and clinical staff for wound management. Perform general nursing duties as needed. Participate in quality improvement plan for the facility. Develops skin care programs to prevent skin breakdown. Demonstrate knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care, and incontinence. Demonstrate revision to plan of care as indicated by the patient’s response to treatment and evaluate overall plan for effectiveness. Formulate a teaching plan based Qualifications: Currently licensed as RN (Required) Wound Care Certified (Preferred) Minimum 3 years’ experience in working within a skilled nursing, short term rehabilitative environment.
