LPN - ACT Team Nurse

Description ACT Nurse (LPN)About your role:As our ACT nurse, you will work as a member of the Assertive Community Treatment (ACT) Team, serving as a critical component of care to adults with severe mental illness in their homes, places of employment, and communities.
