Position Status : Full-time, Day Shift

Position Location: This position is remote for residents of VA

Standard Working Hours : 8:00AM to 5:00PM (ET).

Minimum Requirements:

4 year RN Bachelor Level Degree RN North Carolina, VA (VADHP) or multi-state license 2 years Management experience 3 years Nursing experience 3 years Long Term Care Preferred 3 years Medicaid experience preferred

Job Summary

Serves as the subject matter expert overseeing and directing a team responsible for care coordination activities, routinely assessing and monitoring member status, needs and progress. Accountable for leadership, education, and supporting execution of strategic direction for clinical operations. Ensures quality of care meets or exceeds regulatory standards and requirements as measured by the division clinical quality indicators and contractual agreements. Identifies gaps in transition processes and facilitates development and implementation of solutions emphasizing high quality, positive outcomes in vulnerable populations. Develops and continually drives a culture of clinical excellence, technical competence, compassion and customer-focus within a broad-based interdisciplinary workgroup. Oversees and supports implementation of clinical educational programs for employees, participating practitioners, partners and members. For health plans Clinical Operations, develops and oversees all administrative functions, managing interdependencies and risks, program status and evaluation, program growth/expansion in both new and existing markets. Under direction of Program Director, collaborates with leadership across the organization to achieve clinical outcomes and ensure consistency.

Proven ability to drive clinical teams toward a culture of compliance, strategic and organizational agility, and mastery of engaging and developing staff policies.
Knowledge and experience with regulations and best practices applicable to care management of vulnerable populations.
Extensive knowledge of regulatory requirements and policy coverage guidance applicable to Medicaid/Medicare utilization management.
