Emergency Medicine Clinical Pharmacist 100%

This position is based out of the emergency department. This position will have rotating shifts including weekdays, weekends, and evenings. Clinical pharmacists may assist with coverage as staffing needs dictate. Successful candidates will be required to self-disclose any conviction history and will be subject to a criminal background check and credential/education verification. This position is not eligible for University sponsorship for employment authorization. Clinical pharmacists at UIHC are involved in all aspects of patient care including, but not limited to, therapeutic drug monitoring, medication reconciliation, medication education, and precepting pharmacy learners. This role will support the highest level of safe and effective patient care via thorough clinical review and verification of medication orders. In addition, all members of the pharmacy team collaborate daily with members of the interdisciplinary team, including licensed independent practitioners, medical students and residents, nurses, and social workers, and care coordinators. Position Responsibilities Provides Therapeutic Drug Monitoring (TDM) 1.Evaluate medication orders to ensure accuracy and appropriateness 2.Assure pharmacokinetic assessment of drug dosing on appropriate patients 3.Communicate with prescribers on patient medication needs 4.Identify and evaluate adverse drug reactions on patients 5.Provide drug information to other healthcare professionals 6.Confirm/clarify patient drug allergies noted in medical record when needed 7.Complete medication histories on patients and reconcile home medications against the admission medications 8.Review and reconcile medications upon patient transfer between clinical services 9.Review daily therapeutic monitoring information for patients served by the patient care unit (and other patient care units covered by the pharmacist’s health care team) 10.Identify high-risk medication therapy by defined admission criteria and review as appropriate 11.Complete required competency exams as required by department Education Requirements Pharm. D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy Current licensure by the Iowa Board of Pharmacy Examiners is required. Includes those eligible for obtaining this licensure Experience Requirements One (1) year of applicable pharmacy experience is required. Years of applicable experience include the time spent completing an ASHP-accredited pharmacy residency. Excellent written and verbal communication skills Job related experience with and/or commitment to service excellence Professional experience working effectively with individuals from a variety of backgrounds and perspectives. Desired Qualifications Experience in hospital pharmacy setting is highly desired Basic Life Support (BLS) certification or eligibility for completing BLS certification Advanced Cardiac Life Support (ACLS) certification or eligibility for completing ACLS certification Pediatric Advanced Life Support (PALS) certification or eligibility for completing PALS certification Satisfactory completion of an ASHP-accredited PGY1 residency in pharmacy Satisfactory completion of an ASHP-accredited PGY2 residency in a specific field Clinical experience in providing pharmaceutical care to patients in emergency department or critical care areas Board certification in Pharmacotherapy or another specialized field Application Process: To be considered all applicants must submit a cover letter and resume (under submission relevant materials) that clearly address how they meet the required and desired qualifications.
